Major Depressive Disorder (MDD) is a pervasive medical illness affecting over 280 million people worldwide. Far from being a personal failure or temporary moodiness, depression is a complex neurobiological disorder involving disrupted neural circuitry, neuro-endocrine dysregulation, and neuro-inflammatory processes. Understanding clinical symptoms enables early diagnosis and evidence-based interventions.
Diagnosis requires the presence of at least 5 of the following symptoms during a continuous 2-week period, representing a noticeable change from previous functioning. At least one symptom must be either depressed mood or loss of interest/pleasure:
| Diagnostic Domain | Clinical Manifestation | Patient Experience |
|---|---|---|
| Core Affective Symptoms | Persistent depressed mood; Anhedonia (loss of pleasure). | Feeling empty, hopeless, or numb; previously loved hobbies feel completely pointless. |
| Vegetative & Somatic | Significant weight change; Insomnia or Hypersomnia; Psychomotor agitation or retardation. | Waking at 4 AM unable to fall back asleep; extreme heaviness in limbs ('leaden paralysis'). |
| Cognitive Impairments | Diminished ability to think, concentrate, or make decisions. | Severe brain fog; struggling to read a single paragraph or answer simple work emails. |
| Self-Perception & Safety | Feelings of worthlessness or excessive guilt; recurrent suicidal ideation. | Believing oneself to be a burden on family; persistent thoughts of self-harm. |
